Terms and conditions for FIT4MOM | The Monthly Membership

THE MONTHLY MEMBERSHIP AGREEMENT.

This contract made between the client and Natalie Jean Fitness is a month to month agreement and except as provided in the initial 10 day cancellation policy below, may be terminated by either party with the proper advance notice. This membership allows you to attend unlimited Stroller Strides, Stroller Barre, Strides 360, Body Boost Workout classes. %%INITIAL%%

AUTHORIZATION FOR PAYMENT

I authorize Natalie Jean Fitness to initiate a charge to the credit card number entered into Pike13 for my dues. This authorization is to remain effective until I have canceled my membership through the FIT4MOM franchise owner. I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with Natalie Jean Fitness to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the 3rd day of the month. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a return fee of $25. Members are responsible for updating their card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be canceled. %%INITIAL%%

CANCELLATION POLICY

To cancel this membership, the member must contact the FIT4MOM franchise owner by the 15th of the month via email prior to their next payment due date. VERBAL, TEXT MESSAGE, OR SOCIAL MEDIA NOTICE IS AN UNACCEPTABLE FORM OF CANCELLATION. All membership changes must be made in writing, email Natalie at natalielerno@fit4mom.com, email is the only acceptable form of contact for membership changes. No refunds will be given for dues already paid. This membership is not transferable to another person or anywhere outside the East Contra Costa franchise. %%INITIAL%%

HOLD POLICY

Memberships may be held a maximum of 4 months per hold period. With a max of two holds per year. Hold starts will coincide with the next billing date and can end on any day of the month. When a hold period ends, the monthly auto payments will resume on the return date. There will be a $20 processing fee charged to your credit card on file for the hold period, it is NON-REFUNDABLE. The $20 processing fee will be credited back to your account upon your return as a monthly member. To place your membership on hold, the member must contact the FIT4MOM franchise owner by the 15th of the month prior to their next payment due date. 10-DAY CANCELLATION POLICY

You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement, written notice via email must be received by Natalie Jean Fitness within the initial ten days. You are responsible for paying the registration fee and the length of time of your active contract through your cancellation point (max of 10 days of membership). %%INITIAL%%

FIT4MOM OnDemand MONTHLY SUBSCRIPTION

With your paid monthly membership you are given free access to FIT4MOM OnDemand. You will have unlimited access to OnDemand workouts. When your FIT4MOM membership is properly canceled according to the Terms and Conditions, your OnDemand subscription will be automatically canceled. If you would like to keep your OnDemand subscription you can re-enroll using the same login information and purchase your desired paid subscription. %%INITIAL%%
